Social Prescribing Co-ordinator
* Epsom, Surrey
* £12/hr (Full time 35 hours per week)
* Temp to Perm
The purpose of this role is to lead on fully implementing and delivering a Social and Wellbeing Prescribing service in the borough of Epsom and Ewell. With the aim of increasing the wellbeing of vulnerable groups through directing them to local services, the successful candidates will work as a key part of the primary care network multi-disciplinary team.
Social prescribing helps strengthen personal resilience and reduces health and wellbeing inequalities. It does this by addressing the wider determinants of health, such as debt, poor housing and physical inactivity, by increasing peoples active involvement with their local communities.
Responsibilities:
* To work with GP practices, health and social care providers to implement processes for patient referrals
* Provide personalised support to individuals, their families and carers to take control of their health and wellbeing
* Co-produce a simple personalised care and support plan to improve health and wellbeing, introducing or reconnecting people to community groups and statutory services
* Manage and prioritise own caseload, in accordance with the needs, priorities and any urgent support required by individuals
* To undertake one-to-one appointments at community venues and/or residential homes, providing information on local services to support people
* To undertake or follow up appointments with patients referring to services and feedback back to GPs as appropriate
* Working under instruction, provide a good, all round administration service that supports with the delivery of the health and wellbeing services
* Participate in learning provided to perform the role successfully and to the standards expected, taking an active interest in developing administrative skills
* Meet designated administrative tasks within specified timeframes
* Collate, record and present team data in accordance with the Councils performance measurement systems and share this as requested with other officers and members
* Ensure enquiries are dealt with efficiently and more specialist support is sought from colleagues where appropriate
* Organise meetings including the co-ordination of agendas, attendees, minutes and room bookings
