Belmont Recruitment are looking for an experienced Reablement Practitioner to join Barnsley Councils Adults Social Care Team on an initial 3-6 month temporary contract. This is a full-time role working 37 hours per week, Monday to Friday.

Duties & Responsibilities:

* Undertaking strength based assessments which places the service user at the centre of the decision making process and engages their family

* Undertaking reviews, case management and medication support

* Attending service users homes across the Barnsley area

* Assisting with support plans for service users in order to maximise independence

Requirements:

* Relevant Level 3 Qualification in Social Care

* Experience of working with vulnerable adults

* Full UK Drivers Licence

* Knowledge of care assessments and care management processes
