Tripod Partners are urgently looking to place 2 x Adults Social Workers in the Hospital discharge team to start ASAP.
I am looking for a Social Worker with some hospital work experience. Someone who is able to complete Care Act assessments to determine discharge route from hospital.
Working with the multi-disciplinary team to facilitate discharges
Conducting care home reviews, completing funding panel applications
Carrying out mental capacity and best interest assessment / ensuring clear lines of communication
Knowledge of safeguarding cases ( we have a safeguarding hub) however knowledge of safeguarding to direct cases to the right pathway.
The team is expected to be in the Town Hall one day per week to network with the community team, learn from each other and share knowledge.
Candidate is expected to visit the appropriate hospitals to conduct assessments.
Candidate is expected to be in the host hospital at least once per week (rota system) and when requested to completed their assessments / meet with family members if appropriate.
Essential Requirements:
Degree in Social Work
Hospital discharge experience
2+ years post grad experience
Resides in the UK and has RTW
Registered with SWE
