SW Locums are looking to source a Qualified Social Worker With Social Work England registration and excellent Post Qualification experience within Adult Services in the UK.
We have two vacancies, one in Hospital Discharge and one to undertake Continuing Health care Assessment alongside health colleagues (CHC)
Our client in Knowlsey is in need of a competent worker to provide a professional social work service, including statutory social care responsibilities
To contribute to service delivery that is strengths based and outcome focused with an emphasis on promoting independence and enabling recovery
to work collaboratively with other professionals and agencies to address complex needs and achieve the most ideal outcomes for individuals and their carers that supports their choice and independence and to work with health colleagues to ensure safe and timely discharges from hospital, to the most appropriate service and setting for the individual.
Duties and knowledge required:
* Undertake comprehensive needs-led assessments under the Care Act 2014 and prepare, implementing and the monitoring of care plans.
* Working with service users and their families/carers.
* Attending and participating effectively in supervision by team leader/senior practitioner on regular basis in accordance with Directorate policies.
* Co-working effectively with other professionals including attending multidisciplinary case discussions and team meetings.
* Recording information accurately in line with Council policies.
* Expected to be committed to continuous workplace learning through organisations and attendance at social care forums and meetings.
* Any other duties as may reasonably be required.
* Ensure social care services are commissioned with best value and personalised outcomes in mind and ensure that social care funded packages are reviewed on a regular basis in a robust way.
* To contribute to the commissioning and provision of effective after-care services, including those required under s117 MHA, Care Act 2014 and CP.
In line with government legislation, all colleagues who work in a CQC registered care home or will need to attend such a setting as part of their role must be fully vaccinated against COVID-19.
This role has been deemed in scope of this legislation and therefore to be considered for the role you must confirm on application you are fully vaccinated or will be fully vaccinated by November 11th 2021, or that you are exempt.
Evidence of vaccination or exemption will be required as part of the pre-employment checks Knowsley will undertake on you.
