Belmont Recruitment are currently looking for a Support Worker to join a Local Councils Supported Living Service in the Doncaster area on an ongoing contract. They are looking for people who can work full time, on shifts of either earlies (07:00am-14:00pm), lates (14:00pm-22:00pm) or nights (22:00pm-07:00am) on a block booking basis.

The successful candidate will provide support for adults who have a learning disability and/or associated physical disabilities. They will be responsible for providing high quality care and support to meet the needs of adults at the supported living service.

Key Duties:

* Support tenants with their daily living needs including personal, nutritional, social and emotional needs, in line with personal support plans.

* Support tenants with accessing new activities, opportunities and leisure interests to stimulate and encourage independence.

* Implement the use of outcome based and person-centred support plans to help individuals to make informed choices and decisions, enabling them to clarify and express their needs to achieve outcomes.

* Ensure that any concern about the alleged abuse of an Adult at Risk is addressed ensuring procedures are followed correctly.

* Active involvement with handovers, meetings and tenants reviews, delivering an overview of tenant care where required.

Requirements:

* Experience of working with Adults with Learning Disabilities

* Have UK based support work experience (at least 6 months)

* Have an NVQ or equivalent in Health and Social Care, or similar (desirable)

* The ability to work well both as a team and independently
